Introduction<\/span><\/b> <\/span><\/h2>\n

The <\/span>dislocation of the patella<\/span><\/b> is a common orthopedic pathology, particularly in small-breed dogs. The <\/span>ball joint<\/span><\/b> (or patella) is a bone located in the tendon of the quadriceps muscle, essential to the proper functioning of the knee joint. When the patella moves abnormally out of its normal position in the femoral trochlea, we speak of <\/span>dislocation<\/span><\/b>. This condition can be <\/span>medial<\/span><\/b> (inward dislocation) or <\/span>lateral<\/span><\/b> (outward dislocation). Small breeds such as the Yorkshire Terrier, Chihuahua and Poodle are particularly prone to this condition. Aetiology and predisposed breeds<\/span><\/b> <\/span><\/h2>\n

The <\/span>dislocation of the patella<\/span><\/b> is mainly caused by congenital or developmental abnormalities that affect the alignment of the extensor apparatus of the knee. Factors that may contribute to this condition include: <\/span> <\/span><\/p>\n

    \n
  • Abnormal bone shape<\/span><\/b> Malformation of the femur, tibia or tibial crest.<\/span> <\/span><\/li>\n<\/ul>\n
      \n
    • Shallow or absent femoral trochlea<\/span><\/b> or absent.<\/span> <\/span><\/li>\n<\/ul>\n
        \n
      • Hip anomalies<\/span><\/b> Certain hip deformities can increase the risk of dislocation.<\/span> <\/span><\/li>\n<\/ul>\n

                          Patellar dislocation can also occur in large breeds, although this is less common.<\/span> <\/span><\/p>\n

                          Symptoms of Patella Luxation<\/span><\/b> <\/span><\/h2>\n

                          The <\/span>symptoms<\/span><\/b> of patella dislocation vary according to the severity of the dislocation (classified into 4 grades), and can range from simple discomfort to total inability to use the affected limb. Here are the most common signs: <\/span> <\/span><\/p>\n

                            \n
                          • Intermittent lameness<\/span><\/b> The dog may limp, skip a few steps, then regain a normal gait after shaking or stretching its leg.<\/span> <\/span><\/li>\n<\/ul>\n
                              \n
                            • Cowboy gait<\/span><\/b> Hindquarters may appear to bow inward.<\/span> <\/span><\/li>\n<\/ul>\n
                                \n
                              • Decreased activity<\/span><\/b> The dog may be less active or avoid games requiring rapid movement.<\/span> <\/span><\/li>\n<\/ul>\n
                                  \n
                                • Muscular atrophy<\/span><\/b> Loss of muscle mass in the thigh due to lack of use of the limb.<\/span> <\/span><\/li>\n<\/ul>\n
                                    \n
                                  • Knee pain<\/span><\/b> The dog may show signs of pain when the knee is manipulated.<\/span> <\/span><\/li>\n<\/ul>\n
                                      \n
                                    • Clunking sensation<\/span><\/b> Dislocation of the patella may cause a noise or sliding sensation when manipulating the knee.<\/span> <\/span><\/li>\n<\/ul>\n

                                      If left untreated, the condition can evolve into <\/span>permanent lameness<\/span><\/b> and lead to <\/span>degeneration<\/span><\/b> such as osteoarthritis.<\/span> <\/span><\/p>\n

                                      Diagnosis of patella dislocation<\/span><\/b> <\/span><\/h2>\n

                                      Visit <\/span>diagnosis<\/span><\/b> of patella dislocation is based on clinical examination. Palpation of the knee allows us to assess the stability of the patella and determine the severity of the dislocation. Dislocations are classified into 4 grades: <\/span> <\/span><\/p>\n

                                        \n
                                      • Grade 1<\/span><\/b> The patella can be dislocated manually, but returns spontaneously to its original position.<\/span> <\/span><\/li>\n<\/ul>\n
                                          \n
                                        • Grade 2<\/span><\/b> Patella occasionally dislocates, but can be replaced manually or spontaneously.<\/span> <\/span><\/li>\n<\/ul>\n
                                            \n
                                          • Grade 3<\/span><\/b> The patella is permanently dislocated, but can be replaced by manipulation.<\/span> <\/span><\/li>\n<\/ul>\n
                                              \n
                                            • Grade 4<\/span><\/b> The patella is permanently dislocated and cannot be replaced manually.<\/span> <\/span><\/li>\n<\/ul>\n

                                              From <\/span>X-rays<\/span><\/b> can be taken to assess bone structure, measure the degree of osteoarthritis and plan surgery if necessary.<\/span> <\/span><\/p>\n

                                              Treatment Options for Patella Luxation<\/span><\/b> <\/span><\/h2>\n

                                              The <\/span>treatment<\/span><\/b> of patella dislocation depends on the severity of symptoms and the grade of dislocation. The main treatment options are as follows: <\/span> <\/span><\/p>\n

                                              Surgical treatment<\/span><\/i><\/b> <\/span><\/h3>\n

                                              Surgery is recommended for dislocations of grade 2 or higher, especially when symptoms are present. Surgical techniques include: <\/span> <\/span><\/p>\n

                                                \n
                                              • Transposition of the tibial crest<\/span><\/b> This technique consists in moving the bony prominence where the patellar tendon attaches, thereby realigning the patella correctly.<\/span> <\/span><\/li>\n<\/ul>\n
                                                  \n
                                                • Medial desmotomy and lateral imbrication<\/span><\/b> Medial desmotomy involves loosening the tissues on the side where the patella luxates, while lateral imbrication tightens the tissues on the other side to hold the patella in place.<\/span> <\/span><\/li>\n<\/ul>\n

                                                  • Trochleoplasty<\/span><\/b> This technique deepens the femoral trochlea, providing better anchorage for the patella.<\/span> <\/span><\/li>\n<\/ul>\n

                                                            Prognosis and post-operative management<\/span><\/b> <\/span><\/h2>\n
